Dr Chong Mei Sian is a Senior Consultant geriatrician and Medical Director of The Geriatric Practice. She graduated from National University of Singapore in 1995 and started her Geriatric Medicine speciality training in 2000 after passing her Member of Royal College Physicians (UK) examinations. She obtained specialist accreditation in 2003 and subsequently received training under Prof Jeffrey Cummings in UCLA, USA for cognition (2003), Prof Sonia Ancoli-Israel for sleep in the elderly (2004). She has Graduate Diploma in Clinical Epidemiology (Newcastle, Australia) and was admitted to Fellow Royal College of Physicians (Edinburgh) in 2011., She further underwent training in delirium care (Delirium Room, St Louis University, Missouri USA and Hospital Elder Life Program (HELP)), Harvard Medical School, Boston, USA (2010)) and Massachusetts Alzheimer’s Disease Research Center (2013) (Massachusetts General Hospital and Harvard Medical School, Boston, USA).
Prior to this, she has been practising in Tan Tock Seng Hospital’s Institute of Geriatrics and Active Ageing and Geriatric Medicine department for the past 16 years. During her time there, she helped establish the Subacute Geriatric Unit in 2009 and the Geriatric Monitoring Unit in 2010 which adopted best geriatric practices to develop a novel model of delirium care in the acute setting. This won the National Healthcare Group (NHG) Improvement Project Competition 2013 – Sustainability Award. She was doctor-in-charge of the Cognition and Memory Disorder Service, Centre for Geriatric Medicine from 2010 to 2014. She was Program Director of TTSH-AMK NHGP dementia integration in setting up Primary Care Dementia Clinic from 2012 to 2016 as part of the national efforts to beef up capability to manage persons with dementia. This has since evolved to Geriatric Integrated Network for Dementia (GERInD) in the Central Region, which includes medical integration of general practitioners and geriatric home care team.
She was a Program Director of the inaugural Staff Registrar Scheme (Diploma in Geriatric Medicine in Hospital and Intermediate and Long Term Care (ILTC)) from 2008 to 2010, held in conjunction with Academy of Medicine, Singapore and Chapter of Geriatricians, Singapore. She was also Senior Clinical Lecturer in National University of Singapore and co-Program-Coordinator for Institute of Geriatrics and Active Ageing’s Intensive Practical Course in Delirium/ Dementia Care for healthcare professionals. She was specialist volunteer on the Singapore Volunteers Overseas (SVO) Training Workshop on “Caring for the Caregivers of the Elderly” cum feasibility study for SVO Specialist Team (Community Based Mental Health – Psychogeriatrics) to Republic of Indonesia in Lawang, East Java, Indonesia in 2007 and 2009 with Singapore International Foundation.
Dr Chong was a member of the Ministry of Health National Dementia Network and Chairman for MOH Clinical Practice Guidelines for Dementia in 2007. She also helped formulate the National Dementia Strategy (2008-2009) (internal document) for Ministry of Health, SIngapore. She assisted in formulating the inclusion of Dementia into the National Chronic Disease Management Programme (CDMP) and assisting MOH in the implementation process (2010-2011). She had just completed her part-time appointment with MOH Community Mental Health Division to draft the National Dementia Strategy 2017-27.
She was on the Clinical Scientist Career Scheme (2012-2015) from NHG Singapore with a focus on cognition, frailty, sarcopenia, successful ageing and models of geriatric care. She has published more than 80 papers and book chapters, given many presentations and talks in both local and international meetings.
She is passionate about geriatric care, in particular dementia care. With The Geriatric Practice, she hopes to bring a state of health and well being to older persons as much as possible, and to work with their families to improve the quality of life for older persons and their caregivers. The scope of dementia care spans healthy ageing (functional screening), through geriatric syndromes to attending to the care of older adults in the home setting.
Dr Chong is fluent in English and Mandarin. She also speaks the following dialects (Hainanese, Cantonese and Hokkien).
In her free time, she volunteers at a weekend free clinic and volunteers at animal-assisted activities with her dog.
